Home Health Physical Therapist (PT) – San Francisco, CA

NorthMed Health is partnering with a well-established home health organization in the San Francisco Bay Area to hire a Home Health Physical Therapist (PT). This role offers one-on-one patient care, a manageable territory, and flexibility within a supportive team structure.
This position is ideal for clinicians who enjoy autonomy, meaningful patient relationships, and the ability to deliver high-quality care in a home-based setting without excessive driving or unrealistic productivity expectations.

POSITION HIGHLIGHTS
Setting: Home Health
Location: San Francisco, CA
Schedule: Full-Time, Monday-Friday, rotating weekend call
Territory: Small, well-defined coverage area
Patient Population: Adult and geriatric patients

KEY RESPONSIBILITIES
Provide in-home physical therapy evaluations and treatment
Develop and implement individualized plans of care
Educate patients and caregivers on mobility, safety, and recovery
Coordinate care with interdisciplinary team members
Maintain accurate and timely documentation
Ensure compliance with home health standards and regulations

QUALIFICATIONS
Licensed Physical Therapist (PT) or license-eligible in California
DPT or Master’s Degree in Physical Therapy
Home health experience preferred; training available
Strong time-management and communication skills
Comfortable working independently in the field
